Voluntary Coverage - Evidence of Insurability (EOI)
​As a reminder, EOI forms are required under the following circumstances:

LTD and STD Coverage:​

If you have a previously declined EOI on file.

Voluntary Life AD&D

Employee New entrant: If you elect coverage when you are initially eligible, EOI is required only for any amount over $150,000.

Spouse New entrant: If you elect coverage for your spouse when you are initially eligible, EOI is required only for any amount over $10,000.

Employee Annual Enrollment: Employees who previously elected coverage for themselves can increase their coverage by up to two increments ($20k) as long as the increase does not exceed the maximum allowed amount. If you wish to increase your coverage by more than 2 increments, you must submit an EOI form

Spouse Annual Enrollment: Enrolled spouses can increase their coverage by up to two increments ($10k) as long as the increase does not exceed the maximum allowed amount. If you wish to increase their coverage by more than 2 increments, they must submit an EOI form

Late entrant: Employees/Spouses who previously declined coverage during their initial enrollment or waived coverage at the last annual enrollment can elect up to 2 increments at Open Enrollment.
